Tokyo Hikifune Hospital is making numerous efforts every day to provide good medical care to patients. We believe that it is very important to properly protect and manage the “personal information of patients”. For this reason, we have established the following personal information protection policy and ensure its implementation.

1. Collection of personal information

When we collect personal information of patients, it will be collected for purposes related to medical treatment, nursing care, and patient medical care. When using personal information for other purposes, we will inform you of the purpose of use in advance and obtain your consent. We do the same when we use patient personal information on our website.

2. Use and provision of personal information

We will not use the personal information of patients beyond the scope of the original purpose, except in the following cases. 1) When consent is obtained from the patient. 2) When personal information is used in such a way that individuals cannot be identified. 3) When the provision is required by laws and regulations, etc.
We will not provide the information to a third party without the patient’s permission, except in cases stipulated by law.

3. Proper management of personal information

We will keep the personal information of patients accurately and up to date, and strive to prevent leakage, loss, destruction, falsification, or unauthorized access to the information.

4. Confirmation and correction of personal information

If a patient requests disclosure of his/her personal information, we will confirm the content without delay and respond with our “Personal Information Protection Operational Guidelines”. In addition, if the content is requested to be corrected for reasons such as not being true, we will investigate and respond appropriately.

5. Compliance with laws and regulations and improvement of the system for personal information protection

In addition to complying with Japanese laws and other norms regarding the protection of personal information, we will review each of the above items as appropriate and strive to continuously improve our personal information protection system.

6. Provision of personal information to third parties

We may handle patients’ personal information for clinical research. In addition, we may provide case and test data to third parties for various research purposes, not only inside the hospital but also outside the hospital. We ask for your understanding and cooperation regarding the use of personal information.

7. External storage of personal information

Our medical records, including personal information, are stored in our facility. As a countermeasure against cyber-attacks and to meet the storage requirements required by the “Guidelines for Safety Management of Medical Information Systems” issued by the Ministry of Health, Labor and Welfare, we store backups of our hospital’s medical records externally in a data center via the network.
