Appointment: No appointment is required for first visits.
Before coming to the hospital: download and complete the Medical Consultation form and the Internal Medicine or Surgery Intake form.
What to Bring: Passport, Patient Registration Form, and either the Internal Medicine Intake Form or Surgery Intake Form.

Medication Prescription
Our in-house pharmacist dispenses medicines.
If you have an outpatient prescription for medicine, you will receive the medication at the pharmacy on the first floor.
Please wait in front of the pharmacy.
Payment
We accept the following credit cards in addition to cash for payment of medical expenses. Please present the credit card at the time of payment.
